Upon arrival, staff were asked to identify the names and whereabouts of the children in their assigned group. There were 18 school age children with 3 staff members. A total of 12 children were identified out of the 18. Staff person #1 identified 4 young school age children and 3 older school age children. Staff person #2 identified 5 young school age children. Staff person #3 stated that she was helping but did not have an assigned group yet.

Files for child #1, #2, #3, #4 and #5 were requested on 12/10/2024 to be sent to the Certification Representative. The emergency contact form and agreement form for child #1 dated 12/10/2024 and an immunization record dated 02/20/2024 was received on 12/19/2024, A health report for child #2 dated 12/11/2024 was received on 12/23/2024. No additional files for the children were received for review.
