On two different occasions during the inspection staff had to remove a chunk of foam from a toddler's mouth after they took a bite of the block. Removed during inspection. A cord was observed to be accessible to children in the board room. Cord made in accessible during the inspection.

    The refrigerator in 100R-3 was observed to be set to a temperature of 50 degrees. An open container of rice in the refrigerator drawer appeared to be spoiled as evidenced by the change to a greyish/green color. Rice disposed of during the inspection.
