    Observed staff person #1 in the Panda classroom assisting a child in the bathroom. From their position in the bathroom, staff person #1 was able to supervise 3 children napping nearby but was unable to supervise the remaining 6 children napping in the classroom. 1 child was behind a shelf, and 5 children were on the other side of the room which were blocked from view by the wall in the center of the room. There were 4 older toddlers and 6 preschool age children with 1 staff person in the classroom during naptime.

A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

    The file for staff person #3 did not include complete provisional hire documents prior to their first day working with children (see LIS code sheet). The file included documentation of the FBI fingerprinting receipt dated 08/31/2024 which was after the first day caring for children. Observed staff person #8 and #9 were supervising a group of 16 preschool age children in the Dolphins classroom. When asked to identify their groups, staff person #8 initially named 5 children and staff person #9 identified 7 children and then named 8 of their children. When it was pointed out that there were 16 children in the room and only 12-13 were identified, staff person #9 then properly identified 9 children in their group. Staff person #8 then named 6 children and then realized there were 7 children in their group and were able to properly identify all 7 children in their group. Observed staff person #10 and #11 were supervising a mixed group of 2 preschool children and 8 older toddlers in the Pandas classroom.
