    During the inspection, multiple ceiling tiles were observed to be sagging and water-logged. These ceiling tiles were located in the hallway leading to the classrooms and kitchen. Water spots were also observed in classroom 4. A rug in the back right hand side of the large playroom was observed to have curled edges, creating a potential tripping hazard.

Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. 2. The legal entity will submit their supervision policy, which shall cover cell phone use, including ear buds, to DHS for review. Verified. 3. The legal entity will review the supervision policy with staff and document staff acknowledgement of the policy at the time of review. 4. Legal entity and members of management will conduct and document random supervision checks in classrooms to ensure staff are not on their phones/earbuds. Supervision checks should occur at random times, including naptime. 5. The legal entity must submit documentation of supervision checks in a format approved by DHS. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format.

    During the visit, staff #1 was observed to be wearing earbuds while supervising children. Staff #2 was observed to be looking at a cell phone while children were napping in room 5. Staff #3 was observed to be looking at a cell phone while supervising children in the infant room.

Also in room 10 there was a light panel cover that had evidence of water damage, and still dirty, that has not been cleaned or replaced. In addition: It was observed on 3/25/25, that the kitchen door was open, giving children access to a functional stove/oven.
