The children were then brought to the table to color a picture while the TV remained on in the background. Continued noncompliance previously cited on 07/23/2025, Observed 2 young toddlers and 3 older toddlers in the toddler classroom sitting at the table coloring and engaged in crafts for an extended period of time. Staff were not actively directing the activity for the development of skills and were unable to maintain the toddler's attention. The TV was on at a high volume and caused distraction for the children at the table. The activities were not age appropriate and the children were sitting at the table for a period of at least 1 hour.

A 5th child (an older toddler) arrived at the same time as the certification representative. After observing the drop off process, Certification Representative asked staff person #3 to identify their group, staff person #3 named 1 young toddler and 1 older toddler. Staff person #4 was then asked to identify their group and named 1 older toddler and 1 preschool age child. The older toddler that had entered the room was not identified in either group. When asked about the 5th child in the room, staff person #4 then added the child to their group and named 2 older toddlers and 1 preschool child in their group.

Certification representative then entered the young toddler classroom. Staff person #3 was observed caring for a group of 4 young toddlers and 3 older toddlers. The staff:child ratio with young toddlers in a mixed age group is 1:5. Staff person #3 was over ratio from approximately 9:15 am until 2 children were able to be picked up at approximately 10:05 am. Upon review of the attendance sheets and the number of staff caring for children in the facility prior to staff person #1 arriving, it was determined that staff person #2 was over ratio from approximately 8:45 - 9:12 am with a mixed age group of 2 older toddlers and 9 preschool age children. At approximately 9:15, staff person #2 had a group of 3 older toddlers and 12 preschool age children until staff person #1 arrived at 9:25 am. Staff person #4 was in the facility but was not permitted to be in ratio with the children due to an expired NSOR clearance.

Upon review of the emergency contact and health report for child #1, peaches were listed by both the parent and the child's physician that the child is allergic to peaches and to administer an epi-pen if injested. An epi-pen was not onsite at the facility.

At 9:40 am, observed child #2, an older toddler sleeping in a highchair in the preschool classroom. According to staff, the child had been sleeping since approximately 9:05 am. Staff did not move the child onto age appropriate rest equipment during this time.

At approximately 9:30 am, observed an open bottle of milk on a service cart which was not refrigerated. Staff person #1 stated the milk was brought upstairs for breakfast at approximately 7:00 am. At approximately 9:40 am, the milk was returned to the refrigerator after sitting at room temperature for approximately 2 1/2 hours.
