At the time of inspection, a part of the metal chain linked fence surrounding the outdoor play space was leaning creating a gap between the metal fence post and the building wall which was an entrapment hazard. Also, the double swing gate entrance of the chain-linked fence had a gap at the gate latch post which was wide enough to be a foothold for climbing over the fence.
