It was observed through video footage that on 4/20/2021 that Facility Person #1 left the Infant classroom out of ratio on three separate occasions between 12:30 and 3:30pm. It was observed that Facility Person #1 left the classroom for approximately 1-13 minutes on two separate occasions leaving the other teacher in the classroom alone with 8 infants. It was also observed that Facility Person #1 left the room and came back with an additional child creating an infant classroom ratio of 2:9.
