The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. The portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of in-person training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. Documentation of completed training must be submitted to DHS and be kept on file at the facility. The operator shall provide a date of when this training will be completed.

    On 3/20/26 between 11:15 am and 11:30 am Certification Representative witnessed, staff #2 on the flooring changing a child's diaper, with their back toward a group of young toddlers. Staff person #3 was in the closet. Another staff person, who was in the hall, near the kitchen, could see in the room, witnessed a child climbing up the back of a chair. The staff person from the hall quickly entered the room and removed the child from the back of the chair. Staff #2 and staff #3 were not adequately supervising the children in their group.

    Between 2/6/26 and 3/10/26, staff person #1 would observe other staff persons' changing a child's diaper. Per staff person #1, this is so they could understand the procedure of diaper changing and how to change a diaper. At diaper changing times, staff #1 had their back to the children and was not supervising the other young toddlers or infants in the room. Staff #2 and Staff #3 have had to remind staff #1 to supervise the other children in the room.
