It was observed, large holes in the screen of the sliding door leading to the outside play space. it was also observed that blind cords were dangling and accessible to children resulting in a possible strangulation hazard. It was observed in the outside play space an unlocked shed containing gasoline containers accessible to children, splintered lumber around gate of fence, litter, and pieces of packing Styrofoam across the entire fenced in play area.
