A toddler in the Young Toddler Room was observed with a plastic toy block in their mouth. The block measured less than one inch in diameter. Also, more of these same blocks were observed in the Young Toddler Room and were accessible to the toddlers who were being supervised there.

A Spider-Man backpack located on a cubby shelf in the hallway area outside of the Rainbow Room was observed to have a container of Boudreaux's butt paste that was labeled, "keep out of reach of children." This butt paste was accessible to children.
