In the 2nd-4th grade room, an approximately 4'x2' metal shelf was observed to have a sharply pointed protrusion along the edge of the bottom of the shelf. In the school age hallway women's restroom, an approximately 3'x2' freestanding wooden table was observed to have an exposed nail on the underside of the tabletop. In the K-1st grade room, an approximately 3' round wooden table was observed to have exposed bolts on the underside of the table where each of the four table legs attached to the tabletop. On the door to the girl's restroom on the toddler side of the building, the wooden trim around the vent towards the bottom of the door was broken on one of the four trim pieces leaving a jagged, splintering edge. In the PreK room, an approximately 4'x2' bulleting board framed in aluminum was observed to have a sharp edge at the lower left joint of the framing. All of the above noted areas of concern were observed in areas accessible to children in care, and each presented the risk of causing puncture and/or laceration injuries.
