The outdoor playground is covered in grass and a black unitary shock-absorbing surface. Staff #1 noticed a blister on the bottom of Child #1's left foot when the staff tried to put the child's shoe back on. Child #1 was diagnosed with a second degree burn on the bottom of their left foot, approximately 5cm in length spanning from the 1st to 4th toes and ball of the foot.

Next, Staff person #1 uses the palm of their right hand and slaps child #1 in the face on their left cheek.
