    On 7/2/25 at approximately 2:30 pm, Child #1 was outside in the outdoor playground for approximately ten minutes without a shoe on the child's left foot. The outdoor playground is covered in grass and a black unitary shock-absorbing surface. Staff #1 noticed a blister on the bottom of Child #1's left foot when the staff tried to put the child's shoe back on. Child #1 was diagnosed with a second degree burn on the bottom of their left foot, approximately 5cm in length spanning from the 1st to 4th toes and ball of the foot.

    Based on staff conversations and video footage the following observation were made, on 12/10/24 at approximately 10:50am staff person #1 was positioned sitting on a chair in a face-to-face position with child# 1 in the Older Toddler room. Then Staff person #1 is observed holding a book in their right hand and hitting child #1 on the head with the book. Next, Staff person #1 uses the palm of their right hand and slaps child #1 in the face on their left cheek. After that, Staff person #1 is observed roughly moving child #1 from the circle mat, picks child # 1 up underneath their armpits and placed the child away from the circle mat.
