It appeared that an animal may have dug beneath the artificial grass, removing support underneath the surface and creating a shallow, uneven area. To the right side of the hole was also a raised spot that presented a tripping hazard. There was a fence net hanging off the fence creating spots where a child can get trapped. The wired fence was pulling off the fence pole in the preschool playground. There was a piece of the fence pulling off exposing sharp wired ends.

On 2/26/26 in the infant room at approximately 2:45 pm, an infant was observed in a state of high-intensity cry. The infant's face was flushed, eyes were tightly closed, and the body exhibited significant muscle tension while positioned in an infant activity table/seat. This level of crying had been maintained at a consistent high volume and intensity for a minimum of 10 minutes. During this interval, Staff #1 was present in the room watering plants with their back turned toward the infant. When asked why no attempt was being made to console the infant, Staff #1 stated, "Babies cry." No soothing interventions were observed by Staff #1 during this period.

When asked about the remaining 5 children she responded that they were assigned to staff #3. Staff #3 then entered the room and identified the 5 remaining children.
