    Based on staff interviews, on 5-2-24, Staff person #1 was changing Child #3's diaper on the diapering table and slapped Child #3 on their bare right leg. As a result of the slap, Child #3 had a hand print mark on their leg. Child #3 then was observed rubbing their leg after the incident. Staff person #2 reported that they observed the incident as it occurred and reported it to Staff person #3. There was no incident report written and the incident was not reported for suspected abuse to ChildLine as mandated by the CPSL. Based on staff interviews, on an unknown day, Staff person #1 pulled Child #4 away from the sink in the Toddler A room causing the child to fall and hit their face on the sink. Staff person #1 then proceeded to drag Child #4 across the floor by the child's arm. As a result of the incident, Child #4 sustained a bruise on their face as reported by Staff person #2 who witnessed the incident as it occurred and reported it to Staff person #3. There was no incident report written and the incident was not reported for suspected abuse to ChildLine as mandated by the CPSL. Based on multiple staff interviews and a written statement from staff person #2 who was present in the room at the time, at approximately 9:30am on 5-1-24, Staff person #1 yanked on Child #1's arm so hard that the child's face slammed to the floor causing a bruise on the outside of the child's cheek. It was also reported that as a result of this incident, the child sustained superficial abrasions to the upper and lower lip. Staff person #2 reported that when told about the incident Staff person #3, the director, said to write an incident report that stated Child #1 was participating in circle time and fell thereby bumping their cheek off of the floor leaving a bruise, which was not an accurate description of the incident. Staff person #2 reported that they wrote a written statement describing what actually happened to the child and gave it to Staff person #3. The incident was not reported for suspected child abuse to ChildLine as mandated by the CPSL.
