    At approximately 10:05am Staff Person's #1 and #2 were observed to be caring for 15 preschool age children in Classroom 101 (Elephants). Staff Person #1 identified being responsible for 8 of the children present in Classroom 101 and 1 child who was outside the classroom receiving speech therapy. Staff Person #2 reported being responsible for 6 of the children receiving care in Classroom 101. This resulted in one child, in Classroom 101, not being identified by Staff Person #1 or #2. Child #1, who was receiving speech therapy, was not physically present in the classroom with the staff assigned responsibility for the child. The facility lacked signed parental consent from the parent releasing the child to the therapist.
