The facility self-reported that on 9/11/24, the following incident occurred: Child #1 stuck her finger in the crack of the door in the toddler classroom and the door was shut on it. Child #1 tried to pull her finger out as the door was shutting. This caused the nail to become unattached. On 9/17/2024, this incident was confirmed by a photo of the injury and staff interviews.
