The FBI was not completed until 9/14/2024 and the NSOR request was completed 8/30/2024. Facility staff #2 had a hire date of 9/3/2024. The disclosure statement was signed 8/30/202 The FBI was not completed until 9/13/2024 and the NSOR request was completed 9/4/2024. Facility staff #3 had a hire date of 9/15/2024. The disclosure statement was dated 9/6/2024. The NSOR request was completed 9/12/2024. Facility staff #4 had a hire date of 9/3/2024. The disclosure statement was signed 8/27/2024. The NSOR request was completed 9/4/2024. Facility staff #4 required out-of-state clearances and there was not documentation those requests were made at the time of inspection.

3270.151(a) · A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
At the time of inspection on 9/28/2023, Certification Representative observed that facility staff #1 was alone with children and did not have all required clearances on file. The record showed that facility staff #1 lived in NJ within the past 5 years and there was not out-of-state clearances on file. Facility staff #1 was observed to have their own group of children and during a transition time left a classroom with the group and was not supervised by another qualifying staff person.
