During this time, children were observed standing on lifejackets that were on the pool deck causing a trip hazard where a child could have fallen in the deep end of the pool. Staff #1, #12, #13 and #14 were responsible for direct supervision of the 40 PK children. Staff #12, #13 and #14 were not on the other side of the pool, not able to properly supervise and direct the children away from standing by the deep end. Additionally, on 7/11/24, Staff # 15 stated they were responsible for 6 children while swimming. Staff #15 did not know the names and could not identify the children assigned to their group for the purpose of supervision.
