At 10:00 AM on 6/6/24, observed Child #1 in a Pack 'n' Play in the dining room. At this time, the Pack 'n' Play was directly in front of an air conditioner, which was accessible to the child, a cord was hanging into the Pack 'n' Play, and the Pack 'n' Play was surrounded by other furniture. Also observed a TV in the dining room, which was not mounted, which creates a toppling hazard. In the outdoor play area, observed standing water in a kayak and a ladder propped against the fence, which creates a toppling hazard.

Outdoor play space used by the facility is considered part of the facility premises. On 6/6/24 at approximately 10:00 AM, Staff Person #1 was on the inside porch when Cert Rep arrived. At this time, Staff Person #1 could not see, hear, direct and assess the four unrelated children who were in care in the living room and dining room. Staff Person informed Cert Rep that she had only been on the porch for less than one minute to get supplies.

Staff Person #1 informed Cert Rep that child was being weaned off of binky and was only permitted to have binky in Pack 'n' Play and had been in the Pack n Play for approximately ten minutes. Cert Rep informed Staff Person #1 that this was considered restraining in a confined space.
