Observed in the living room near the entrance: Electrical devices, back chair massager, and air filter, which create entanglement and toppling hazards. At 10:00 AM on 6/6/24, observed Child #1 in a Pack 'n' Play in the dining room. At this time, the Pack 'n' Play was directly in front of an air conditioner, which was accessible to the child, a cord was hanging into the Pack 'n' Play, and the Pack 'n' Play was surrounded by other furniture. Also observed a TV in the dining room, which was not mounted, which creates a toppling hazard. In the outdoor play area, observed standing water in a kayak and a ladder propped against the fence, which creates a toppling hazard.

Staff Person #1 informed Cert Rep that child was being weaned off of binky and was only permitted to have binky in Pack 'n' Play and had been in the Pack n Play for approximately ten minutes. Cert Rep informed Staff Person #1 that this was considered restraining in a confined space.
