When staff were asked again to identify and name the children they were responsible for, Staff person #2 required assistance from Staff person #1 to identify the children in their supervision group. At approximately 11:30 AM, Facility Persons #3, #4 and #5 were exiting the gross motor room with 2 older toddlers and 9 preschoolers when they stopped to identify their supervision groups. After each staff person named their children, it was discovered one of the preschoolers was not identified by any of the staff as a part of their supervision group. In total 10 of the 11 children were claimed by a staff person as being supervised by them.
