In the Infant Room, in the cabinet above and to the left of the sink nearest the diapering station, two bottles containing unknown liquids (one a blue liquid that smelled of cleaning solution and one a clear liquid that smelled of bleach) were observed. Neither bottle was labeled with its contents.

3270.76 · Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
In the infant room, the sink nearest the diapering station had an area of rust near the faucet. The rusted area was sharp, creating a potential hazard. In the Toddler One room, there was an area of rust on the sink that was flaking. In the bathroom adjoining the Toddler One and Toddler Two rooms, rust was observed in the doorway and on the shelf above the sinks. Both areas of rust showed evidence of flaking onto the surfaces below. In the Pre-K bathroom, a toilet paper holder was observed to have a rusted base, as well as rust near the top where toilet paper is attached. In the Young School Age room near the storage closet, a broken bracket was observed on the wall. The bracket had a sharp bottom which could pinch or scratch a child. In the Outdoor Play Space, a pile of splintered wooden pallets was observed to be leaning against the fence. A hose was observed to be coiled up in a pile in the outdoor play space, creating a potential strangulation hazard. Also in the outdoor play space, tall weeds were observed to have thorny edges, which could injure a child.

3270.82(h) · A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
Handwashing signs were not observed at the following locations in the facility: in the infant room by the diapering area, diapering sink and food prep sink; in the Toddler One room by the diapering area; in the Toddler Two room by the food prep sink.

3270.102(a) · Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
In the Outdoor Place space, the following toys and outdoor equipment were found to be in poor repair: a white storage bin near the windows had a crack near the lid that created a sharp edge; on the embedded play equipment, there was peeling rust and paint on the walkway; in the covered play space, the plastic kitchen, car and parking toy were dirty and cracked.

3270.102(c) · Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.
The outdoor play space included embedded play equipment with a protective surface covering visible; however, the facility records did not include documentation that the surface meets ASTM standards for poured-in-place equipment. Additionally, several areas were observed to have missing pieces of that protective covering, leaving the surface in poor repair.
