Staff will not use physical punishment towards a child and staff will not single a child out. The part of the plan shall have an immediate correction date. 2. Director will create a supervision policy that address physical punishment and singling a child out. All staff will be trained and sign off on the new supervision policy. The part of the plan shall have a correction date of when this will be completed.

During a complaint inspection on 6/20/25, Rep viewed video footage from 6/16/25 which shows staff #1 deliberately tripping child #1. Staff #1 also admitted to tripping child # 1 to "slow child down". Staff# 1 no longer works at the facility.
