A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. Tiered LIS: 1. Staff will not use physical punishment towards a child and staff will not single a child out. The part of the plan shall have an immediate correction date. 2. Director will create a supervision policy that address physical punishment and singling a child out. All staff will be trained and sign off on the new supervision policy. The part of the plan shall have a correction date of when this will be completed.

    During a complaint inspection on 6/20/25, Rep viewed video footage from 6/16/25 which shows staff #1 deliberately tripping child #1. Staff #1 also admitted to tripping child # 1 to "slow child down". Staff# 1 no longer works at the facility.
