    During a complaint investigation started on 8/18/23, it was determined that on 8/15/23 at 3:16 PM, Staff person #1 left Child #1, a preschool aged child, unsupervised in the classroom while the rest of the group went outside. A parent found the child alone in the classroom at 4:10PM.

    During a complaint investigation conducted on 6/15/21, a written statement was obtained from Staff Person #1. In the written statement, Staff Person #1 wrote that on the morning of 6/4/21, Child #1 was arguing and continuously bothering another child in the class. Staff Person #1 said that after asking Child #1 to sit down & keep his hands to himself, Child #1 ignored her directions so she grabbed his bookbag, which was on his back, to get his attention and he fell back. During the complaint investigation, Staff Person #2 told cert rep that during an internal investigation done by the facility, Staff Person #1 admitted to pulling Child #1's backpack loop. Staff person #1 apologized to Staff Person #2 and said that she was sorry & knew it wasn't the right thing to do
