A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person # 1, # 2, # 3 and # 4 may not work in a childcare position at the facility. The CPSL was revised on 9/18/2018 to include a requirement that all clearances be updated and on file at least every 60 months.

    During a complaint investigation started on 10-4-23, staff person #1 admitted that she did not notify the Regional Office within 24 hours of Child #1 receiving emergency room treatment on 9-21-2023. She also admitted that she didn't submit a written report to the Regional Office within 72 hours of same incident with Child #1 receiving emergency room treatment.
