3270.76/3270.77(a) · Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
A renewal inspection occurred at the facility. A physical site review was conducted. The outdoor play area contained a large wooden swing in need of repair. The swing had screws/nail that were coming loose and a small area on the seat that had a little dry rot. The arms of the swing also contained peeling paint. Children were observed playing in the area that contained the swing during this inspection.

An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Staff #2 was hired using a Department of Education FBI clearance dated 1/14/23. The CPSL indicates this is not an acceptable clearance to work in a Department of Human Service program.
