In the school age classroom, paint, borax, cleaners, and a buck of cleaning water were observed accessible to children. In the preschool classroom, Miracle Gro Potting Mix and in the bathroom of this classroom scrub free toilet cleaner were both observed accessible to children. All of these items indicate the need to be kept out of reach of children. During this inspection these areas were observed to have children utilizing the spaces with the toxic items.

Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The legal entity must develop or update their written supervision and ratio policies to be implemented in the facility. The written policies must address how the facility will maintain appropriate child to staff ratios at all times. The policy should include but is not limited to topics that deal with maintaining child to staff ratios when one staff needs to leave the classroom area for any reason. The legal entity will submit their policies to the Northeast Regional Office for approval. Once approved, all existing staff and any new hires must be trained on the approved policies and it must be documented in their files.
On 1/3/24 at 10:28 am, staff person #1 was observed supervising 14 preschool aged children by herself in the preschool aged classroom that is located by the main entrance doorway. Staff person #1 was observed to be alone for approximately two minutes until staff person #1 was asked by staff person #2 to rejoin the group.

The CPSL was revised on 9/18/2018 to include a requirement that all clearances be updated and on file at least every 60 months. The Child Protective Services Law (CPSL) was revised to include the requirement for the National Sex Offender Registry (NSOR) Clearance effective 9/30/2019. The CPSL also states that anyone hired after 9/30/19 was required to have the NSOR clearance to work in child care. Current staff must have the NSOR clearance no later 7/1/20. CPSL was revised to include new hires that have resided out of Pennsylvania with in the 5 previous years must obtain State Police and Child abuse clearances from the state that they have resided.
Staff person #1 did not update her criminal, child abuse, and FBI clearances within 60 months. The original criminal clearance was dated 6/17/22, and it was not updated until 7/14/22. The original child abuse clearance was dated 6/14/17, and it was not updated until 6/16/22. The original FBI clearance was dated 6/12/17, and it was not updated until 7/13/22
