A staff person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #2 may not work in a childcare position until all completed paperwork is on file at the facility.

Staff person #2 (see code sheet for date of hire) only had one child abuse clearance on file, dated 2/8/2023, and was missing the previous clearance. Staff person #2 also had state police clearances on file dated 1/2/2018 and 2/8/2023, and DHS FBI clearances on file dated 1/12/2018 and 2/13/2023. These clearances were not completed prior to the previous ones expiring.

During inspection on 4/12/23 interviews and document review revealed that the facility does not have a record of conducting fire drills every 60 days. During inspection on 5/17/18 document review and interviews revealed that the plan of correction and plan to maintain compliance was not completely implemented. Fire drill log was not posted by leadership staff and was not posted on a portable board, so it is visible to those who need to see it and access per operator plan of correction and plan to maintain compliance.
