A renewal inspection occured at the facility. A physical site review was conducted. In the infant room, a bottle warmer was found accessible to children. The bottle warmer contained a warning to keep out of reach of children and infants/toddlers were observed receiving care in this room at the time of the inspeciton. Staff indicated they had recently used the warmer to warm up a bottle.

The OCDEL Announcement C-22-06 requires all current child care staff who completed the previous health and safety pre-service training modules prior to October 4, 2022 to complete the Pennsylvania Health and Safety Update 2022 by December 30, 2022. Staff # 1 did not complete this training until 2/14/23.

Dirty and dusty vents, ceiling tiles and light covers were observed in: Shining Stars, Bright Stars, Discovery Preschool and Sunflower 2. Peeling paint was found in the bathroom of Bright Stars. A hole was found in the bathroom of the Discovery Preschool. All areas listed were identified and addressed with the staff at the time of the inspection.

Per inspection at facility on 3/22/23 it was revealed that on March 3, 2023, child #1 fell while outside on the playground causing an injury to his lip. Staff #1 did not complete and provide an injuries report for the parent on the day of the accident.

Staff person #-1-2-3-4-5-6-7-8-9-10-11 & 12- have acceptable pediatric first aid/CPR training in an approved curriculum; however, the trainer has not provided documentation that they are PQAS-certified in that training topic.

3270.132(a) · If emergency medical care is needed for a child, the parent shall be contacted as soon as practical in the best interest of the child. If the parent cannot be reached, the operator shall record in writing the reason emergency care was required and the attempts made to inform the parent.
On 6/22/22, Child #1 fell and hit his face causing damage to his teeth and requiring emergency medical care. The incident occurred at approximately 11:30am. Parent was not notified of the incident until approximately 1:15pm despite child's repeated complaints of pain.
