A signed and dated disclosure statement must also be completed. Staff #7 was missing documentation they applied for their FBI clearance and the file was missing a completed NSOR and disclosure statement. Staff #5 was missing their completed child abuse clearance, NSOR clearance and a completed disclosure statement. Staff #5 and #7 were working together at the time of this inspection, unsupervised, and were not hired correctly per the CPSL. Per the CPSL, to be hired as a provisional hire, staff must apply for their state police clearance and receive their completed NSOR clearance and disclosure statement. Staff #6 was missing documentation they applied for their state police clearance and did not receive a completed NSOR and disclosure statement. Per the CPSL, to be hired as a provisional hire, staff must also apply for their state police clearance/FBI, receive one and be waiting on the missing clearance to be hired as a provisional hire. Staff #8 did not apply for or receive one of the state police clearance/FBI, did not receive the completed child abuse clearance, NSOR or disclosure statement. Staff #6 and #8 were working together at the time of this inspection, unsupervised, and were not hire correctly per the CPSL. Per the CPSL, to be hired as a provisional hire, staff must apply for their state police clearance and receive their completed NSOR clearance and disclosure statement. Staff must also apply for their state police clearance/FBI, receive one and be waiting on the missing clearance to be hired as a provisional hire.
