    CONTINUED NONCOMPLIANCE An unannounced inspection occurred at the facility on 6/9/25. A physical site review was conducted. In the large main space, a bottle of A&D ointment was found in a child's cubby that was accessible to children, but the bottle indicated the need to be inaccessible to children. Children were observed receiving care in this area at the time of the inspection. In both rear bathrooms of the building, the cabinets were unlocked and contained Lysol and soaps. These bathrooms were observed being used by children at the time of the inspection. These items indicated the need to be inaccessible to children. The gate to the kitchen was left open and accessible to children were several disinfectants, paint, and cleaners. These items indicated the need to be inaccessible to children. Children were observed receiving are in the room directly attached to the kitchen's open gate. A door labeled 'Employees Only' was left unlocked. In this room the following toxic items were found: cleaner, bleach, floor stripper, laundry detergent and a mop bucket with dirty water and cleaner (water was gray and smelled of cleaner). Children were observed passing by this unlocked door that contained toxic item that the labels read to keep out of reach of children. An acceptable plan of correction was received 7/16/25. A renewal inspection occurred at the facility on 8/14/25. A physical site review was conducted. In the large main space, numerous bottles/containers of diaper rash creams were accessible in the diaper changing drawer and in the children's cubbies. In the small left room, diaper creams were also accessible in the diaper changing drawers. Children were observed receiving care in these rooms and the items found indicated the need to be kept out of the reach of children. These items were discussed with the director at the time of the inspection.

Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff hired as of the date of the training to receive a minimum of three hours of in-person, PQAS-approved training, regarding supervision and appropriate ratios of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. To show proof of the training, the legal entity will submit the staff attendance/sign-in sheet to the Regional Office after completion of the training. The operator shall provide a date for when this training will be completed.

    CONTINUED NONCOMPLIANCE An unannounced inspection occurred at the facility 6/9/25. A physical site review was conducted. In the 3rd large room from the entrance, a preschool age girl was observed unsupervised. It is unknown how long this girl had been in this room. At this same time, Staff #6 appeared to be looking for a missing child. Certification Representative asked Staff #6 if they were missing a child and Staff #6 admitted they were. This child was reunited with their group in the 3rd large room from the entrance with Staff #5. On a second occasion, Certification Representative was walking through the facility with the director and at that time, another preschool age child was found alone and unsupervised in the same section of the building. It is also unknown how long this child had been gone from Staff #5, as Staff #5 did not appear to be looking for the missing child. The director reunited this child with Staff #5, who appeared unaware that the child was missing. An acceptable plan of correction was received on 7/16/25. A renewal inspection occurred on 8/14/25. A directed plan was given to the facility, and an acceptable plan of correction was given that the facility would arrange for all facility staff hired as of the date of the training to take PQAS-approved training for supervision. As of the inspection on 8/14/25, this training has not been taken.

The Legal Entity/Director will establish a supervision and mixed-age ratio policy. The policy will include how the facility will demonstrate their knowledge and ability to adequately supervise children while always maintaining appropriate ratio during times when mixed-aged children are grouped together. The policy must address the process of determining proper staff:child ratio when mixed-aged children are grouped together. The policy must be submitted to the Northeast Regional office for approval. Staff #3 and Staff #6 were observed working in the large main area with 3 older toddlers, 8 preschoolers and 1 school age child. Staff #3 was observed leaving the room on two occasions with 1 child leaving Staff #6 with 2 older toddlers, 8 preschoolers and 1 school age child. This number of children would have required at least 2 staff to be present to meet staff:child ratio when Staff #3 left the room on both occasions.     An unannounced inspection occurred at the facility. A physical site review was conducted. The facility had a strong unpleasant smell. Found in the second large room from the entrance there were 2 bags of garbage that staff indicated was found at the exit door at the opening of the facility on the date of this inspection. The kitchen is directly attached to this room and the kitchen too had a strong unpleasant smell. Another garbage can was found with the lid open and filled with garbage. Staff could not say when this garbage was last taken out. Staff and children in care on the date of this inspection all commented on the strong unpleasant smell.

    An unannounced inspection occurred at the facility. A physical site review was conducted. In the 2nd large room from the entrance, a spilled colorless liquid from a bottle was observed on the floor near the windows. This area was addressed with the director at the time of this inspection. In the 3rd large room from the entrance to the right side near the exit door, in several areas, the flooring was observed peeling up. These areas were identified and addressed with the director. Peeling paint was also observed in this room. The peeling paint was identified and addressed with the director at the time of this inspection. In the last room in the rear of the building, peeling paint and a hole in the wall was found. These areas were also identified and addressed with the director at the time of this inspection. The second to last bathroom to the rear exit of the building was observed to have water leaking but the origin of the leak was unknown. The leak was causing the flooring of the bathroom to peel up and water could be heard under the flooring when walked on. Areas of water also began pooling.

Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff hired as of the date of the training to receive a minimum of three hours of in-person, PQAS-approved training, regarding supervision and appropriate ratios of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. To show proof of the training, the legal entity will submit the staff attendance/sign-in sheet to the Regional Office after completion of the training. The operator shall provide a date for when this training will be completed.

    An unannounced inspection occurred at the facility. A physical site review was conducted. In the 3rd large room from the entrance, a preschool age girl was observed unsupervised. It is unknown how long this girl had been in this room. At this same time, Staff #6 appeared to be looking for a missing child. Certification Representative asked Staff #6 if they were missing a child and Staff #6 admitted they were. This child was reunited with their group in the 3rd large room from the entrance with Staff #5. On a second occasion, Certification Representative was walking through the facility with the director and at that time, another preschool age child was found alone and unsupervised in the same section of the building. It is also unknown how long this child had been gone from Staff #5, as Staff #5 did not appear to be looking for the missing child. The director reunited this child with Staff #5, who appeared unaware that the child was missing.

A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child. TIERED LIS: 1. The legal entity/director shall develop a diaper changing policy to include hand washing, cleaning of the diaper changing area, and proper disposal of the diapers. The policy shall be sent to the Northeast Regional Office for review and approval. Once approved, the policy will be reviewed with all staff. Staff will sign off on the policy indicating their understanding of the diaper changing policy and procedures. The operator shall provide a date for when this process will be completed with all staff.
