3270.27(f) · The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency.

During the renewal process that began on 1/28/22 the inspector reviewed the emergency plan and noted that there was no proof to substantiate that the emergency plan was sent to local municipality and to the county emergency management agency as is required.

3270.135(b) · Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

While at the facility unannounced on 10/20/21 the inspector found a diaper changing mat, on the changing table, in the toddler room, to be ripped and the foam exposed rendering it unable to be cleaned or sanitized.
