LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 7 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

    Staff person # 7 has has a FBI clearance that is over 5 years old. They did apply for a new clearance but have not received it yet. On 7/28/22 during renewal inspection, they were observed working in a classroom. They were not alone. They were with a permanent staff person.

    On 7/21/22, during a video inspection, diapering of a child was observed. The diaper was left on the changing table while the new diaper was put on and not immediately placed in the trashcan. In addition, the child was allowed to walk away from the changing table without having their hands washed.
