At the time of the facility inspection, there was a broken soap dispenser in the children's bathroom. The soap in the case was on the sink, a broken part of the dispenser was on the sink and the broken attachment was on the wall. All of these things were assessable to the classroom children.

The thermometer in the facility refrigerator used to store food for the children's meals was at 51 degrees at the time of the inspection. There was also spillage on the bottom of the refrigerator.
