During the renewal inspection on 3/14/25, during the afterschool program, the certification representative called the facility to gain entrance. Staff person #2 acknowledged that they were by themselves and could not leave the room. The certification representative was escorted by Bradford Heights staff to the afterschool classroom. Staff person #2 was observed alone with 14 children. No other staff persons were present at the facility.

During the renewal inspection on 3/13/25, staff person #1 was unaware that child #1 walked out of the library and into the hallway, prior to transitioning from room #146 (room in the back of the library) to the Gym.

During the renewal inspection on 1/30/24, staff person #1 and #2 did not have assigned responsibility of specific children. The staff indicated they were both responsible for all children.
