The updated health reports for Child #2 and Child #4 were received more than 12 months after the last one. For Child #2, the updated physical was dated 11/28/22 and the one prior was dated 6/15/21. For Child #4, the updated physical was dated 3/1/23 and the one prior was dated 11/10/21. The file for Child #3 did not include an updated health report and the last one was dated 2/4/22.
