During interviews with staff members on 8/13/2025, Staff persons #2, #3, and #4 all reported that they witnessed Staff Person #1 (DOH see LIS code sheet) have Child #1 (DOB see LIS code sheet) in a "bear hug" to restrain child #1's movements. Staff person #3 reported that approximately 3 weeks prior, they observed staff person #1 have child #1 in a bear hug. When staff person #3 told staff person #1 to "Let (child #1) go" staff person #1 replied, "If I let them go, they think they can do whatever they want." Staff person #4 reported that on 7/29/2025 while up in the Chapel, staff person #1 restrained child #1 by wrapping their arms around the child and stated "I had to restrain (child #1) because they were uncontrollable."

It was observed on 7/28/2023 Child #1 (DOB see LIS code sheet) and Child #2 (DOB see LIS code sheet) were sleeping in cribs on their stomachs. When staff were interviewed Staff Person #1 stated that they placed each infant on their stomach to sleep, which is not the sleeping position recommended by the American Academy of Pediatrics.
