Title VII of the Civil Rights Act of 1964 (42 U.S.C.A. §§2000e-2000e-5), Section 504 of the Rehabilitation Act of 1973 (29 U.S.C.A. §794), the Americans with Disabilities Act of 1990 (42 U.S.C.A. §§12101-12514).

During the process of renewing the Certificate of Compliance, the Certification Representative contacted the provider on several occasions requesting that the Civil Rights documentation be emailed to the DHS office or to the Certification Representative. As of today, the Civil Rights documentation has not been received.

During initial inspection on 6/2/2023, the outdoor play space had grass and weeds almost a foot high. The area in the back of the yard was covered with leaves a few inches deep. There was a barbecue grill in the play area.
