During the unannounced renewal inspection on 8/29/25 and 9/16/25 the inspector reviewed the staff files and found that staff #1had a previously dated pediatric first aid/CPR dated for expiration on 4/12/24 and did not renew the pediatric first aid/CPR training until 5/15/24, staff #3 had a previously dated pediatric first aid/CPR dated for expiration on 4/13/24 and did not renew until 5/15/24, staff #4 had a previously dated pediatric first aid/CPR dated for expiration on 4/16/24 and did not renew until 5/15/24, and staff #6 had a previously dated pediatric first aid/CPR dated for expiration on 4/12/24 and did not renew until 5/16/24.

Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.
During the unannounced renewal inspection on 8/29/25 and 9/16/25 the inspector reviewed the staff files and found that staff #1through staff #6 had fire safety training on 8/16/24 and all completed an unapproved fire safety training on 8/18/25.

3270.151(a) · A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.
During the unannounced renewal inspection that occurred on 8/27/24 the inspector reviewed files for staff #1, #2, and #6 and found that all had gone over every 24 months on the required health assessment updates.

Facility Person # 1 and # 2 may not work in a child care position at the facility.
A renewal inspection occurred at the facility. A review of staff files was conducted. See the code sheet for the dates of hire. Staff # 1 did not have an NSOR clearance in their file and did not apply for the DHS FBI clearance prior to working in child care. Staff # 1 was working at the facility on the day of this inspection. Staff # 2 did not have in their file documentation that they applied for the DHS FBI clearance or the NSOR clearance prior to working at the facility. \ Staff # 1 and Staff # 1 were not hired correctly per the Child Protective Service Law (CPSL).

Staff # 1 was hired using a volunteer state police clearance dated 9/6/19. Staff # 1 did not apply for the appropriate state police clearance until 11/3/22. Staff # 1 worked with children before obtaining the appropriate state police clearance. Staff # 2 was hired as a provisional hire. As of the date of this inspection, Staff # 2 has not obtained the NSOR clearance. Staff # 2 was working in a child care role at the facility past their 45-day provisional hire period.
