During renewal inspection, nap time ratios were reviewed. It was observed that the staff in the Preschool room was positioned in a location where she could not see, hear, assess or direct the children on the opposite side of the room. This was due to the fact of bookshelves and additional furniture impeding on her line of site. This staff member was the only staff in the room at the time of nap time.

Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must create a written supervision policy regarding how supervision will be maintained throughout the day and during naptime. The legal entity will need to have all staff review the new policy, sign a document stating that it has been reviewed and it shall be kept in each staff file. This policy must be approved by DHS before implementation. The operator shall provide a date for when this policy will be completed and reviewed by all staff.
A complaint investigation was conducted on 8/25/25. During interviews, Rep was provided with a picture of Staff# 1 laying on a cot and facing the wall. Rep also took photos of the entire Primary A room in which this incident occurred. From the view of the room and position in which Staff #1 was in the picture provided, she would not have been able to accurately supervise the remainder of the children since she was facing the wall. Interviews with staff and director indicate she would have been the only staff in the room since this occurred during nap time.

While at the facility unannounced on 5/18/22 for the renewal visit the inspector observed the outdoor play space to be in disrepair. The tiles in the outdoor play space were lifting up from the ground and were creating a tripping hazard. The vinyl fencing was leaning outward and pitched to an extent that it falling was of concern. The left inside of the fencing had pulled apart, pulling the screws out, creating gapping between the two pieces. This space was rendered unsafe and as such it cannot be used until such time that these issues are fully corrected.
