During Allocated Unannounced Monitoring Inspection on 02/11/2026, Certification Staff observed that Staff Person #1 did not possess a current NSOR Clearance in the Staff Record.

During Allocated Unannounced Monitoring Inspection on 02/11/2026, Certification Staff observed that the Fee Agreement Forms for Children #1, #2, #3, #4, and #5 did not include a date of admission. The Fee Agreement Form for Child #4 did not include a fee amount or date in which the fee is to be paid.

3270.15(a) · A certificate of compliance will not be granted by the Department until the legal entity provides a certificate of occupancy as proof of compliance with the applicable requirements of the Department of Labor and Industry in 34 Pa. Code § 403.23 (relating to child day care facilities) and under section 1016(c) of the act (62 P.S. § 1016(c)).

During Initial Inspection on 12/08/2025, Certification Staff observed that the facility was not yet granted a Certificate of Occupancy.
