    During a Complaint Investigation on 01/05/2026, Certification Staff determined Staff Person #1 did not complete a written incident report immediately following an incident in which a child obtained an injury while in care at the facility. Staff Person #1 did not provide the parent of the child with the original incident report on the same day that the incident had taken place.
