    3270.113(a) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

    On 2/12/25 during the renewal inspection, Staff person #1 was unaware of the number of children were present in the lunchroom due to a child being allowed to go to the bathroom unsupervised. When CR asked how many children were present for the day, Staff person #1 said 13 and the 14th child returned from the bathroom unsupervised.

    On 2/12/25 during the complaint investigation, it was stated that Child #1 was released by staff person #1 to school personnel who was not a designated pickup person that was provided by the parent.

The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

    On 5/2/23, during the renewal inspection, substitute staff was not aware which children were in the programs care at the time of the transition from school departure, bathroom, and into outdoor play space. Lead teacher did not communicate the children in care, which groups, and ratio.
