The files for staff persons #1, #2 and #3 were observed to lack written evaluations of staff persons on a regular basis, a minimum of one evaluation every 12 months. This was evidenced by no evaluations present in the file for staff person #1. Staff person #2's file had evaluations dated 09/19/2024 and 10/19/2025, a period of 13 months. And the file for staff person #3 was observed to contain an evaluation dated 05/08/2024.
