Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. 2. The Legal Entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision/counting of children. The Legal Entity must receive DHS approval of the training content prior to scheduling the training. Certification rep will send a referral to the ELRC to request training.
At time of investigation on 7/16/25, video footage showed and staff person #1 and #2 acknowledged that on 7/11/25 child #1 was left unsupervised on the playground for approximately 5 minutes while the class transitioned inside to the classroom. Staff persons #2 and #3 did not realized the child was missing from the group until child #1 was brought back to the classroom by staff person #1.

During unannounced inspection on 9/5/24, due to self-reported non-compliance, staff person #1 acknowledged that child #1 was left unsupervised in the classroom for less than 1 minute while staff person #1 and the rest of the class exited the room to the playground. Once outside, staff person #1 did a count and realized child #1 wandered out of line and remained in the room.
