At time of inspection on 10/1/24, the following physical site items were observed: stained ceiling tiles were observed in the Primary 2, Toddler 2, and indoor play space. The cabinet doors under the sink in the play space were damaged at the bottom and slightly crumbling when touched. The bottom of the exit door in the Toddler 1 room was visibly rusty.
