A parent of another enrolled child at the facility found child #1 walking alone in the hallway towards the lobby of the facility without an adult and returned child #1 to the gymnasium. Staff person #1 was unaware that child #1, who was assigned to their supervision group, walked without a staff person into the hallway and was missing until the enrolled parent returned the child to the gymnasium and reported the child alone and unsupervised.

3270.113(a) · The legal entity must arrange for pre-approved supervision training for all staff hired as of the approval of the Plan of Correction. The legal entity must receive DHS approval of the training content prior to scheduling the training. Proof of training for each attendee and a list of all currently employed staff will be submitted to the Regional office upon completion of the approved training.
Child #1, a young toddler, was alone and unsupervised for a period of 5 to 10 minutes after becoming locked in the storage closet off the gym. Staff person #1, whose supervisory group Child #1 was assigned to, as well as all other staff present in the gym, were unaware of the whereabouts of Child #1 during this time. There was no staff person present in the storage closet to fulfill the staff to child ratio requirement during this time.

Staff person #4 and Staff person #5 left Child #1 outside in the playground for approximately 1 minute when returning to the classroom on 6/17/21, at about 5:00 PM. No staff were physically present with Child #1 during this time. Staff person #1, #2, and #3 left Child #2 in the preschool room for approximately 25 minutes when they went to the gym, on 6/17/21, at about 3:00 PM. No staff were physically present with Child #2 during this time. During an unannounced inspection on 7/12/21, in the 3 yr old classroom, there were 13 preschool aged children with in the room with Staff person #2 and Staff person #3. Staff person #2 claimed responsibility for 5 PS children and Staff person #3 claimed responsibility for 4 PS children, leaving 4 PS children with no staff person assigned to supervise them.
