Prior for leaving for break, staff #2 told staff #1 that both children were in the bathroom. Staff #1 was patting the back of a child for nap and did not supervise the children in the bathroom. After an unknown length of time, staff #1 went to the bathroom and found the children touching each other inappropriately.

On 3/16/23, at approx.1:45 pm, facility staff #1 roughly placed child #1 on a nap cot as child kept pushing his cot into the wall. At approx. 2:44pm, Facility staff #1 pushed child #1 out of a chair and dragged the child across the room then placed child in a chair across the room. At approx. 3:00 pm, child #1 ran across the room and began to throw pillows. Facility staff #1 picked up child #1 aggressively and carried child under one arm as staff #1 went for the phone. Facility staff than used her foot/leg to block child against the wall. As a result, child #1 received a bruise to the lower back that resembles an adult handprint.
