    3270.111(c) · 1. Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding managing children's behaviors and deescalating children's behaviors. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a date of when this training will be completed.

    On 10/3/25, between 2:40 pm and 3:15 pm, child #1 was removed from the room by staff #1 for fighting twice with another child. Child #1 was taken to the office in a bear hug by staff #1 while child's behaviors were escalated. Child was not allowed to leave the office and child's behaviors continued as child #1 threw a water cooler jug, papers, color pencil and a table at staff #1. Child #1 was able to exit the office, where child #1 proceeded to hit other children and throw papers around the room. Child #1 was carried back to the office by staff #1 and staff #2. A second time, child #1 was able to exit the office again and was escorted back to the office by staff #3 by the collar of the shirt/neck area. Child's behaviors and escalation lasted for a period of 45 minutes until mother arrived to pick child #1 up.
