The legal entity will confirm continuing compliance by sending copies of the facility staff scheduled weekly on Friday prior to the following work week. The legal entity will provide documentation on Mondays as to which facility persons worked the previous week at the facility. The documentation will be sent to the Central Region Office for a period of Three (3) months. The correction date for this portion of the plan shall be a minimum 3 months from the date submissions begin.
On 12/23/25, the facility reported on 12/19/25 between 7:00 am and 8:30 am, 1 staff was present with more than 2 children. On 1/7/26, the facility reported on 1/5/26 and 1/6/26 between 3:00 pm and 6 pm 1 staff was present with more than 2 children. On 1/30/26, the facility reported on 1/28/26 1 staff was present with 7 children between the hours of 3:45pm to 5:45pm and on 1/30/26, 1 staff member was present with 10 children between 3:45pm to 5:20pm. The Facility Director must attend the next Existing Provider Training offered by the Central Region Office of Child Development and Early Learning. A Trash can was observed in the child care space. The school age program did not have a specific trash can dedicated to the program and the trash cans did not have a lid to prevent contamination by human secretions. A lidded trash can is in line with federal requirements administered by the U.S. Department of Human Services, Office of Child Care (OCC) and its requirement for all school-age programs providing care exclusively to school-age children certified by the Department of Human Services (DHS) under 55 Pa. Code Chapters 3270 and 3280 to follow certain guidelines pertaining to the handling, storage, and disposal of hazardous materials and biological contaminants.
