3270.14/3270.21 · A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. A renewal inspection was conducted on 10/8/28. Staff person #1 did not update their Child Abuse and FBI clearances every 60 months as required by the CPSL. This is evidenced by the previously documented Child Abuse clearance on file dated 9/16/19 and FBI clearance on file dated 9/9/19. Staff person #1's current Child Abuse clearance is dated 10/11/24 and FBI clearance is dated 10/1/24. All other clearances are on file. Staff person #2 has not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated 7/2/20. Staff person #2 did complete a Little League mandated reporter training on 4/2/25, however, the mandated reporter training does not meet the minimum of three hours, is not from an approved source and is not intended for an appropriate target audience.

A renewal inspection was conducted on 10/8/25. Cert Rep observed in the downstairs classroom, plastic gloves and plastic wipes on the changing table and the red and blue play mats had rips on all four corners exposing the stuffing inside. Both items are accessible to children that are still placing objects in their mouths.
